Die Zusammenstellung der Website update-coronavirus.ch richtet sich an medizinische Fachpersonen, die in kurzer Zeit einen Überblick über die wichtigsten aktuellen wissenschaftlichen Publikationen im Zusammenhang mit COVID-19 haben wollen. Die Website hat keinen Anspruch auf Vollständigkeit noch wird mit der Auflistung eine wissenschaftliche Würdigung oder Gewichtung des Inhaltes vorgenommen. Sie widerspiegelt die persönliche Einschätzung der Mitglieder der Taskforce.

Die Studie bestätigt den klinischen Alltagseindruck, dass Kinder mit Grundkrankheiten bei mit COVID-19 hospitalisierten Patienten übervertreten sind. Die Interpretation ist allerdings dadurch erschwert, dass das Altersspektrum junge Erwachsene bis 24 Jahre einschliesst und letztere quantitativ übervertreten sind. Weitere Studien sind notwendig, um pädiatrische Risikofaktoren für schwere Verläufe zu definieren.

Ein Rückgang der Antikörper über die Zeit ist häufig und scheint, wie auch Seow et al. bereits in einer kleineren Kohorte berichteten, massgeblich von der Stärke der initialen Immunantwort mitbestimmt. Welchen Einfluss dieser Rückgang auf das Risiko einer Reinfektion hat, ist zum aktuellen Zeitpunkt noch nicht klar. Diese Studie unterstreicht zudem nochmal, dass eine negative Serologie einen vorgängigen Kontakt mit SARS-CoV2 nicht per se ausschliesst.

Hohe Antikörpertiter mit neutralisierender Aktivität für eine Krankheit, wo die humorale Immunität wichtig erscheint (zumindest ist die Impfung zu 90% schützend und hier überwiegt wohl der humorale Effekt der Impfung). Und trotzdem scheinen die entscheidenden Schritte der Krankeitsgeschichte zum Zeitpunkt der Infusion bereits abgelaufen.

Es ist unüblich Pressemitteilungen zu referenzieren und zu kommentieren. Hier wurde eine Ausnahme gemacht. Die zweite Mitteilung zur guten Schutzwirkung eines mRNA basierten Covid-19 Impfstoffes (nach dem Pfizer Produkt) ist ein wichtiger Meilenstein. Auch wenn auf wenigen Fällen beruhend (11 zu 0), legt die Mitteilung auch nahe, dass der Impfstoff auch gegen eine schwere Covid-19 Erkrankung schützt. Die Studienteilnehmerinnen werden weiterhin nachverfolgt, und die finalen Resultate zum Impfschutz können sich von den hier gegeben unterscheiden.

Die Studie konsolidiert mit in vitro Daten eine verbreitete Hypothese zur Erklärung der relativen Protektion von Kindern vor schweren COVID-19 Infektionen. Kinder und Jugendliche erkranken sehr häufig mit banalem Schnupfen durch andere Coronaviren, was partielle humorale Kreuzprotektion vermittelt. Die Daten sind auch bezüglich Impfstoffentwicklung interessant, weil neutralisierende humane Antikörper gegen die S2 Untereinheit kein "antibody dependent enhancement" induzierten.

Dieser enorme Effekt von Social Distancing Massnahmen auf die Epidemiologie praktisch aller relevanten Atemwegserreger in Korea deutet an, dass nicht-pharmakologische Massnahmen die Saisonalität von viralen und bakteriellen Atemwegsinfektion massiv beeinflussen können. Das hat Implikationen für die allgeine Infektionsprävention, besonders für Risikopopulationen.

Endlich ein ausführlich beschriebener Bericht über diese Studie, die in UK, Brasilien und Südafrika durchgeführt wurde. Allerdings bleiben immer noch einige Fragen offen, da weniger als 10% der Probanden älter als 70 Jahre alt waren. Zudem wurden hier noch keine separaten Wirksamkeitsanalysen nach Altersgruppen berichtet, sollen aber später folgen, wenn die Resultate für alle 24'000 Probanden vorliegen.
